You can contact Dr. Zarfati via email or cell phone to schedule an appointment. She will get back to you within 24 hours to schedule an appointment. You will be asked to fill out some basic forms prior to your scheduled appointment. The initial evaluation will be 45 minutes, in which a thorough history will be obtained. She will discuss your goals, needs and aspirations in order to formulate a treatment plan that is tailored to your needs.
INSURANCE & FEES
Dr. Zarfati does not accept insurance. However, she will discuss with you the reimbursement given by your insurance plan and will provide you with the appropriate receipt and billing codes to submit to the insurance provider, so you can obtain the maximum allowable amount from the insurance provider.
